In addition to the requirements and terms agreed to by the adopter in the Standard Adoption Agreement, I hereby additionally agree that I will bring this cat to Advanced Pet Care in Clarkston, MI within thirty [30] days of the above adoption date, to establish a record of care for the animal.

In addition, an ongoing "wellness" plan or equivalent insurance must be secured [with proof in writing] within that same timeframe to ensure adequate ongoing care.

If the above do not take place within the specified time frame, you will be considered to be in breach of the terms of this agreement, and ownership of the cat will immediately revert to Black Cat Society.
