In addition to the requirements and terms agreed to by the adopter in the Standard Adoption Agreement, I hereby additionally agree that I will bring this cat to Pawsitive Care in Holly, MI for the (above) specified number of follow-up visits.

At the conclusion of the follow-up visit(s), Pawsitive Care can provide ongoing care directives to you, the owner, or coordinate future care directly with your preferred clinic or veterinarian.

I understand that the cat I am adopting has a medical condition which requires some degree of continued care from the veterinarian who began the treatment. This is to ensure the cat's progress and health can be evaluated by the vet/clinic who are most familiar with the history and details of the condition, which provides the best possible outcome for the cat.

I understand that I am required to make the above number of visits/appointments at Pawsitive Care to establish a record of timely and appropriate veterinary care, as there is not an existing record of care established for my pet ownership.
