In addition to the requirements and terms agreed to by the adopter in the Standard Adoption Agreement, I hereby additionally agree that this adoption is currently a "Foster-to-Adopt" basis, which requires the both the Previous Guardian [Black Cat Society] and the adopter [you] to agree on the following points of rights and/or responsibilities :

• the Guardian will provide medical care at no cost to the Adopter, until such a time as they determine the animal hereunder is healthy, in no danger, and ready to be released to the owner for care under their discretion and expense.

• the Adopter will agree to take the animal ONLY to the Guardian or their assigned veterinary partners [Pawsitive Care] to receive medical care, until a time that the Guardian signs a Release of Care

• the Adopter agrees to allow a home visit from the Guardian, at a mutually agreed time within 7 days from such request by the Guardian.

• If any agreements above are not fully honored by Adopter, or if Adopter fails to respond to any communication attempts by Guardian within 48 hours of message transmission, the Adoption Contract is considered void and ownership of the animal will be automatically reverted to the Guardian

All terms on this addendum supercede and replace those on the Adoption Contract when specified; all other terms of the Adoption Contract remind in effect.
